To my eyes, it looks as if there was no cordon, with anybody going anywhere they liked. The Miltia fired on the building, which in turn set the terrorists off, and then the Russians started firing. The Militia ended up shooting the Russian forces by mistake.
They did it the British way, that is to say, no aggression on their behalf. But when the suicide bombers blew themselves up (now speculated that the leader killed them), it all went pear shaped. Unlike the British way of doing things, they had no plan of action in case something happened. When it did, they had no direction; no command.
The Russians couldn't get through the doors, as they were blockaded, instead having to climb through the windows. If you look, they are going through the windows very slowly. This is because there were children on the other side covering the entire floor.
In theory, there should have been a quick response team ready to go at a moments notice, much like our SAS. In this case there wasn't.
But then again; these were people that had not really given much thought to escaping, hence the bombs.
