It asks 'What Is your religion?' Shall I just put I havn't got a religion?
Where do I find my National Health No.?
Where abouts on my Birth Certificate is my Birth Certificate No.? Theres a couple of numbers.
And the Expiry date on my passport just says 08 MAY, shall I just put that or do I need a year on it?

If you have any problems just leave it blank and question it with your AFCO personel, they will be better placed to advise you.

right, all that is sorted now but i have made a mistake on the form!
when it asked where my parents were born i put my dads birthplace for my mum and my mums birthplace for my dad!
Shall i just cross it out or put an arrow inbetween them?
Would the navy mind this or would they see it as a lack of concentration?

If it was anything like the forms i filled in, i had to rewrite it in several times before it was right.
As i said before the best people to advise you on what is required on the form are your local AFCO.
It was a good few years ago when i did my forms and the format has probably changed since then.

Right... Fill in the form you have in pencil, anyting your unsure about put a question mark next to it, if you have the information its asking for i.e. passport date, then take thr information with you and ask the question at the AFCO.
They can supply a new application form, the AFCO are their to answer any questions and problems you are having.
If you fill the form in the way i have said above it means you should have all but the blanks filled in when you see the AFCO, and would probably only have to make one trip!
