help please

i got given this form (attestation/engagement form and notice paper for entry into the naval service) i cant remember what my afco said either take it to CTC with me or send it back completed to them. any help would be grat as i have my PRMC on the 17th of may cheers.

Mike, make like ET and use the phone, better to hear an exasperated sigh in the earpiece than a raging instructor in your face demanding to know why the form was not sent early.
3p of phone call or 40 pressups, it's not a difficult choice.

Follow El Prez's superb advice.
Send it to the AFCO along with (IIRC) 4 passport photos, next of kin form and I think something else...
