The move comes as a voluntary United Nations protocol on young personnel, signed by Britain in September 2000, is due be put before Parliament after Easter.
Although it looks like it wasn't until 2002 that it actually became enforced.

Go to the doctors, explain your situation and firmly request a referral to a physio and podiatrist. I did this and got an appointment within a fortnight. Different PCTs though, so it varies from place to place. Otherwise do what illustrious said, first go through the proper channels and if that does...

Unfortunatly mate, that's a bit beyond my area of expertise! I'm sure there is a difference and that different sugars ellicit a different hormonal response, but if your muscles are decently sensitised to insulin then I think that it comes down to personal preferance/availability. But as a quick answer to your question of what you could reasonably acheive: everyone is different. There is no set amount I'm afraid.
